SUN-874 - An Educational Model Improved Diabetic Panel Performance in A Resident Continuity Clinic

Background
Effective diabetes management in primary care requires high-quality panel-based care. At Greater Baltimore Medical Center, residents often struggle to meet diabetes quality benchmarks despite participating in a one-week, dedicated continuity clinic with their own patient panels. Key challenges include 1) residents’ unfamiliarity with panel management and metric goals; 2) infrequent office time (once every six weeks) and underutilization of multidisciplinary resources and 3) gaps in practical skills with diabetic pharmacotherapy. We implemented an education-focused model to improve both resident competency and the quality of diabetes care.
Methods
To introduce the core concept of panel management, residents were provided a goal-oriented, diabetic panel dataset one week prior to their clinic rotation. The dataset included panel-level metrics with benchmark goal of ≥ 80% and patient-level data on the most recent hemoglobin A1C, blood pressure, dates of last dilated eye exam and urine microalbumin test, and the date of next visit. This allowed residents to assess overall panel performance, identify patient-specific care gaps, and develop proactive action plans.
To maximize residents’ clinic learning and multidisciplinary experience, we implemented a structured meeting during clinic week in which residents met with supervising attendings, a pharmacist, and care coordinator team to review panel data and formulate collaborative care plans for complex cases. The meeting also aims to motivate residents by fostering healthy internal competition and peer learning. Besides, residents led multidisciplinary morning huddles to reinforce daily care plans and worked with case manager to schedule ophthalmology appointment during patient visit.
Third, the attendings and the pharmacist organized small-group didactic sessions focusing on the practical aspects of diabetic care, such as initiating continuous glucose monitoring, prescribing insulin, medication choice and navigating insurance coverage.
Results
After one year, improvements were observed in diabetes quality metrics. Comparing aggregated data from January 2025 to December 2025, the proportion of patients achieving glycemic control (HbA1c < 9%) increased from 72.6% to 79.7%, approaching the benchmark goal of 80%. Completion rates for annual dilated eye examinations increased from 46.0% to 58.0%, and annual urine microalbumin test increased from 83.2% to 89.7%. Blood pressure control ( < 140/90 mmHg) remained near benchmark.
Discussion
The improved clinical outcomes suggest enhanced residents’ competency in panel management, multidisciplinary care, and diabetic pharmacotherapy. This educational model addresses common challenges in resident diabetic panel management and may be adaptable to other resident clinics. Future iterations may emphasize order-sets integration and between-visit ownership.
