SAT-644 - An Insulin-Independent Estimate of Insulin Resistance Identifies Clamp-Defined Insulin Resistance in A Multiethnic Adult Cohort

Objective
Insulin resistance (IR) is a central feature of type 2 diabetes, but impractical to assess directly in routine clinical and population-based settings. We evaluated an insulin-independent estimate of IR against the hyperinsulinemic-euglycemic clamp (HIEC) in a multiethnic adult cohort.
Research Design and Methods
In this cross-sectional study, 98 adults aged 18-60 years underwent standardized HIEC. IR was defined as an M-value ≤ 4.7 mg·kg⁻¹·min⁻¹. The Single-Point Insulin Sensitivity Estimator (SPISE), derived from BMI, triglycerides, and HDL-cholesterol, was analyzed in its inverse form (SPISE-IR = 10/SPISE) and compared with established fasting surrogate indices using receiver operating characteristic analyses. Threshold optimization, multivariable modeling, and decision curve analysis were performed.
Results
SPISE-IR demonstrated excellent discrimination for HIEC-defined IR (AUROC 0.90, 95% CI 0.80-0.98). A threshold of 1.80 identified IR with 88% sensitivity and 90% specificity.
Conclusions
SPISE-IR may provide a pragmatic, insulin-independent approach for identifying IR in ethnically diverse adult populations.
